Practice transformation resource for diabetes care updated

The National Diabetes Education Program at the National Institutes of Health's National Institute of Diabetes and Digestive and Kidney Diseases has updated its resource, Practice Transformation for Physicians and Health Care Teams, to provide clinicians with models and processes of care delivery to help enhance the quality of diabetes care for various health care settings.
